Tolnitch Surgical Associates will file your insurance at the time of service; however, copays, deductibles and coinsurances are due at the time of service. Additionally, patients are ultimately responsible for all balances. If your insurance does not pay within 60 days, then the patient will be responsible for all charges. If we do not take your insurance, then full payment is expected at the time of service (unless other arrangements are made prior to the appointment). As a courtesy, we will file insurance for you.

There is a $50 charge for all appointments cancelled less than 24 hours in advance. There is a $25 charge for any surgery or procedure that has to be rescheduled.
